Chronic Conditions Outcomes Framework Workshop 4: Regional

The Department of Health is developing the framework to improve the lives of people living with, and those at risk of developing, chronic conditions in Western Australia.  This workshop is aimed at people who live and work in Regional and Rural WA to gain their insights and experiences with Chronic Conditions.
